STAR is seeking a Care Finder (Aged Care Navigator). The purpose of the position is to provide specialist and intensive assistance to help clients understand and access aged care services and connect with relevant supports in the community.
The ideal candidate will be a current resident of the SMBI island community or have extensive experience working on the islands. They will have previous experience in a Community Services industry (preferably Aged Care). The position will commence at 0.6FTE Increasing to 0.8FTE from 01.07.2023.
